COTI X StaTwig Case Study: 10 Million VaccineLedger Transactions. Zero Data Exposed.

In June 2025, we announced our partnership with StaTwig, the global supply chain technology company behind VaccineLedger — a UN-recognized Digital Public Good and winner of The Trinity Challenge.
Today, we’re sharing the results of what that partnership has produced: a completed pilot integration between VaccineLedger and COTI’s blockchain that demonstrates privacy-preserving cold chain monitoring at scale, live in production.
This isn’t a proof of concept on a testnet. This is over 10 million real blockchain transactions, processing real sensor data, protecting real vaccines, in a live government health program.
Why Vaccine Cold Chains Need Privacy
Vaccines are temperature-sensitive. A single equipment failure or temperature excursion can render an entire batch ineffective, leading to waste, public health risks, and eroded trust in immunization programs.
Traditional monitoring systems store temperature logs in centralized databases, which can be tampered with or deleted. Sharing that operational data with auditors, donors, or regulatory bodies raises serious confidentiality concerns. And as immunization programs scale, the volume of sensor data grows exponentially.
The challenge: How do you create tamper-proof, auditable records of vaccine cold chain and supply chain monitoring without exposing sensitive operational details on a public blockchain?
That’s exactly the problem COTI’s Garbled Circuits were built to solve.
What Was Built
The pilot was deployed in Bangladesh under the Expanded Program on Immunization (EPI) at the Directorate General of Health Services (DGHS). VaccineLedger already serves as the country’s Electronic Vaccine Logistics Management Information System (eVLMIS), tracking over 100 million vaccine doses annually for 65 million beneficiaries.
COTI’s blockchain was integrated into VaccineLedger’s Cold Chain Module to deliver:
Real-time IoT sensor data collection from Wi-Fi-enabled sensors on refrigerators, freezers, and cold rooms at EPI headquarters in Dhaka.
Encrypted data transmission where all sensor data is encrypted at the application level before it ever touches the blockchain. Sensitive information is never exposed in plaintext.
Privacy-preserving alert logic powered by COTI’s Garbled Circuits. Business rules for temperature breach detection, humidity monitoring, and equipment status are evaluated directly on encrypted data.
Immutable audit trails with hourly Merkle tree roots stored on-chain, providing cryptographic proof that historical data has not been tampered with.
12-tier alert system from minor warnings to critical alerts for prolonged excursions or equipment failures, all aligned with WHO Prequalification System standards.
How the End-to-End Data & Privacy Works
The architecture is elegant in its simplicity. IoT sensors continuously monitor temperature, humidity, and battery status on vaccine storage equipment. That data flows through VaccineLedger’s ingestion layer, where it’s validated, enriched with asset metadata, and encrypted using COTI’s encryption framework.
Once encrypted, the data is submitted to COTI’s smart contracts, which use Garbled Circuit logic to evaluate alert conditions directly on the encrypted inputs. If a temperature threshold is breached, the contract emits an event. VaccineLedger’s event listener picks it up, and facility managers are notified in real time.
Every hour, a Merkle tree is built from all the sensor readings for each asset. The Merkle root goes on-chain, creating an immutable fingerprint of the data. Auditors can independently verify that nothing has been altered, without ever needing to see the raw operational data.
The critical detail: at no point does sensitive data appear in plaintext on the blockchain. Even though COTI operates as a public chain, the privacy guarantees are equivalent to a private system.
The Results
Over a one-month pilot beginning in December 2025, the system processed over 10 million blockchain transactions at a total cost of approximately 300–400 COTI tokens. At current value, the system processed 10-million transactions at a cost of under $5 USD.
That’s a cost-per-transaction that is orders of magnitude lower than traditional blockchain platforms, and even centralized server solutions, making the solution economically viable for large-scale national deployments.
After initial optimization, the system achieved near-zero error rates with stable, high throughput. The alert system successfully detected temperature excursions and equipment issues in real time, enabling rapid response to prevent vaccine spoilage. Hourly Merkle roots were verified against on-chain records throughout the pilot, confirming complete data integrity.
The integration was reviewed and validated by Bangladesh’s public health department and officials, meeting international standards for pharmaceutical supply chain monitoring.
Why This Matters for COTI
This pilot is one of the first real-world applications of Garbled Circuit technology for healthcare supply chain monitoring. It demonstrates exactly what COTI was built for: enabling sensitive data to be processed on a public blockchain without ever being exposed.
The results validate COTI’s core value proposition across several dimensions:
Performance at scale: 10 million transactions in a month with near-zero error rates proves that COTI’s infrastructure can handle high-volume, real-world workloads.
Cost efficiency: Processing 10 million transactions over the course of 30-days for less than 500 COTI Tokens or $5 in USD. Unrivaled cost efficiency or any system, and the economics that makes blockchain practical for large-scale privacy infrastructure.
Privacy that works: Sensitive health data remained fully encrypted throughout the entire pipeline. Public auditability without exposing confidential details.
Real World Use Cases: Blockchain has far more use cases than finance, and this pilot is proof. Healthcare, supply chain, compliance — COTI’s privacy layer opens the door to all of them.
What’s Next
StaTwig is already deployed across multiple countries, tracking over 100 million doses annually. With this pilot complete, the COTI integration is positioned to scale across Bangladesh’s entire cold chain network and expand to additional countries as StaTwig and VaccineLedger grow their footprint to over 25 nations in the coming years.
For COTI, this is a clear signal: privacy-preserving blockchain infrastructure isn’t theoretical. It’s live, it’s validated, and it’s ready to scale.

In blockchain, some early projects such as SingularityAI, Fetch.ai and others explored the intersection of AI and blockchain but it was the explosion of generative AI made tangible via ChatGPT in 2022 that formed an inflection point for public awareness.
Then more recently, in the past year we’ve seen the rise of AI trading agents available to retail audiences on exchanges like Bankr Bot on Base and DeFi platforms such as PriveX on COTI.
But as the landscape evolves and AI moves from executing instructions to making intelligent decisions, the ability of agents to process data securely without compromising sensitive information becomes critical. Without robust privacy-preserving computation, AI cannot operate effectively in trading, financial, governmental, or enterprise contexts.
AI and Blockchain: The Future in Beta
Let’s be very clear about the current trajectory. AI agents are evolving extremely fast and very quickly becoming capable of running the whole show for us. Not just thinking, but executing and learning as they go, becoming all roles within a business, and conferring amongst themselves. Get ready for agent to agent business where the human is no longer needed. As one recent viral post on X notes, we are just the ‘beta testers’.
In this scenario, Agents need fast payment rails, they are not going to hook into bank API’s! For a start the settlement times are just not feasible. Agents can execute trillions of operations per second. They are not going to wait 2–5 days for an international settlement. Not to mention the KYC and AML requirements. Agents will naturally favour the fastest and most efficient route to getting business done.
However, as we watch the disruption of AI unfolding, or what Vitalik Buterin has referred to as an era of ‘chaos’, one thing is very clear, AI Agents need privacy. They need cryptographic infrastructure to handle confidential payments at scale and to protect their strategy and IP from others.
Vitalik comments that the future of AI on Ethereum is about how AI systems are designed, who controls them, what data they touch, and whether people can trust them. He has repeatedly argued that AI should work with decentralized systems in a way that protects users, preserves privacy, and avoids concentrating power in the hands of a few actors.
As AI agents start moving on-chain, making decisions, interacting with protocols, and transacting with one another, privacy stops being a “nice to have” and becomes a hard requirement. That’s where confidential computation is essential, and it’s needed today.
As of today, just one platform offers AI Agents with Privacy: PriveX on COTI and that platform is generating a daily volume of upwards of $250 million. We’re early.
COTI Enabling the On-Chain AI Revolution

What is Arb Fast Lane?
The Arb Fast Lane is Bancor’s onchain arbitrage trading bot and execution system.
